how do manual treadmills work?

i use automatic treadmills at the gym everytime but a manual treadmill sounds quite bizzare. how does it work?

Are manual treadmills just as effective as motorised treadmills?

I’m thinking of buying a manual treadmill rather than a motorised one but are they worth it? Would I end up with the same results I would get from a motorised treadmill?
